Hi Connie. To be clear, Gretel was never “down”. She experienced pain and early IVDD symptoms so I took her to the vet right away. However, she was in some pain. She ate fine while on crate rest. Her most favorite thing in the world is to eat! That being said, she won’t eat treats or anything if she is super stressed out. Most dogs won’t eat or drink if they are in significant pain or really stressed out from their environment or something that’s happening to them. Did you try something else besides your pup’s regular food? Some people have luck with a mixture of chicken and rice. I would just keep trying until you find something he will eat and maybe consider talking to the vet about increasing his pain medication (I’m assuming you’ve taken him to the vet and they put him on medication). When grade 0 neurological deficits persist beyond 24-48 hours the result of any treatment (surgical or nonsurgical) becomes minimal. Medical management has been shown to be as ineffective as surgical therapy in the majority of patients with sensorimotor paralysis for more than 24-48 hours. However, some clients do not consider euthanasia as an immediate alternative in these cases and may request some form of therapy.

Treatment options include corticosteroids, enforced rest (cage confinement), acupuncture and surgery. The ideal option depends on the location of the IVDD, whether the dog is a small or large breed, severity and duration of the clinical signs and recurrences. Most surgeons agree on several general treatment protocols. Most small and large breed dogs with cervical IVDD are treated initially with corticosteroids, enforced rest or acupuncture. Surgery is reserved for patients with more severe clinical signs (intractable pain and or motor dysfunctions such as staggering) or recurrent episodes.

Treatment depends on the severity of the signs but is divided into surgical or non-surgical.  It depends on the severity of the signs, previous episodes, whether the disease is in the neck or back, how long the problem has been going on (the longer the problem the less chance of successful surgery) and of course economics.  For dogs with mild back pain a conservative approach with crate rest and medications may be elected.  Dogs with more severe signs (grade 2-5) or persistent back pain that does not respond to pain and rest are best managed with surgery to decompress the spinal cord.
